(1) Your completed health questionnaire  (download below)
(2) Water

Make sure you arrive a few minutes early for your first workout.

NOTE: It is very important not to arrive late to your first workout,
so please 
plan accordingly allowing yourself a few extra minutes
in case you get lost 
or have some type of unexpected delay.
